Complaints can be raised from the Complaints report tile on the dashboard. If this report tile isn't visible you can use the 'Add Reports' button at the bottom of the dashboard and find it from there. To add the report tile permanently to your dashboard you can press the star in the top right of the tile.

Capture details of the reporter.
